Please complete an annual permission form for your teens to participate in middle school and high school youth ministry events. 

Annual Youth Form

Having annual forms on file replaces permission forms for youth events and reduces the amount of information needed when registering for events.  Note that some events will require additional permission and registration forms such as NCYC, OBOC, and Summer Daze.
